TLC Family Health is an affordable membership model of Primary Care, AKA Direct Primary Care (DPC), which works outside the world of insurance and provides you with direct, easy access to your health care provider (through short wait times, portal communication, and emergency cell phone access). We also have a direct relationship with our lab and suppliers, thus removing costly middlemen and drastically driving down costs. Every test we order or treatment we give you is AT COST, which is shockingly low! More about TLC Family Health

Founded by Douglas M. Cluff, M.D. in October 2017, TLC Family Health is now one of over 2000 Direct Primary Care (DPC) practices nationwide (up from just 140 in 2014), and we strongly believe it is the future of Primary Care.  Kicking insurance out of primary care opens up a whole new world, as summarized by our Explainer Video above. TLC Family Health is Primary Care the way it should be, but until now couldn’t be because of insurance, which should be reserved for emergencies (click HERE for recommendations), similar to auto insurance. You don’t expect auto insurance to take care of maintenance needs and most repairs. Similarly, health insurance needs to stay out of Primary Care.


DPC can be a great blessing for the self- or uninsured, in addition to business owners who cannot afford to provide insurance for their employees.  However, most of our patients have insurance, but have either realized that their deductibles far exceed our yearly membership fee (and they get a much bigger bang for their buck by joining our practice) or they really enjoy all the perks!  Also, HSAs may be used for the monthly membership fees.

At TLC Family Health, one of our primary goals is to help you achieve health through lifestyle.  In fact, “TLC” stands for “Therapeutic Lifestyle Changes” – especially Nutrition and Weight Management (our specialties), Fitness, Stress Management, and Sleeping Well.  We do our best to help you stay or wean off medicine; however, we recommend it when we feel it is best to use.  Come join us on our three-fold mission, and see what magic we can conjure up for you!

how it works:

At TLC Family Health, you receive all labs, tests, and injections at cost (usually >90% off usual prices, e.g., a cholesterol panel is $3.25; a strep test is $1.25; a B12 shot is $2.94), which removes any incentive to add unnecessary tests or services!  This drastically reduces overall costs, at the same time improving the patient-physician relationship and quality of care. 

At TLC Family Health, you have two membership options (both of which provide the same premium level of care – see below for pricing). The two membership options are:

  1. Gold Plan (90% off visits and communications, along with other premium discounts)

  2. Silver Plan (50% off provider visits and communications, along with other discounts) - NOTE: See “SILVER PLAN EXCLUSION CRITERIA” below.

  • Registration fee:  $100 for first person; $150 max for 2+ family members

  • Receive 5% off if you sign up for 6 months, and 10% off if you sign up for 12 months

SILVER PLAN EXCLUSION CRITERIA: Note that most DPC practices do not offer a second, lower-priced plan with a higher copay. The reason is that the higher copay disincentivizes care – and one of the primary goals of DPC is to remove barriers to care. Dr. Cluff has chosen to offer a Silver Plan for the sole purpose of providing an option for those who rarely need to be seen, and therefore cannot justify the price of the Gold Plan. This opens the door so that a larger percentage of patients can experience the many benefits of TLC Family Health. That being said, certain medical conditions require the Gold Plan because of the higher level of care needed (more frequent visits are needed in order for us to manage them correctly, and therefore the copay/barrier to seeking care must be lower). These include:

  • Diabetes

  • Uncontrolled hypertension

  • Body mass index >35

  • Age >45
